Soccer Lesson Liability Waiver and Policy

Acknowledgment of Risks:
Participant/parent acknowledges that participating in soccer lessons involves inherent risks, including but not limited to, falls, collisions, and contact with other players or equipment, which may result in injury. Participant acknowledges an understanding that injuries can occur despite the best safety measures.

Voluntary Participation:
Participation in soccer lessons is entirely voluntary. Participants have the opportunity to ask questions regarding the safety of the activities, and their decision to participate is based on their own assessment of the risks involved.

No Liability:
Participants release, waive, and discharge Kick Start Academy, its coaches, assistants, and any affiliated parties from any and all claims, demands, or causes of action for personal injuries and property damage that may occur as a result of my participation in soccer lessons.

Medical Conditions:
Participants certify that they do not have any medical conditions that would prevent their safe participation in the soccer lessons. If they do have any conditions that may affect their ability to safely participate, they will inform the instructor beforehand.

Assumption of Responsibility:
Participants assume full responsibility for themselves/any minors registering for these lessons, including any risks of injury and agree to follow all safety rules and instructions provided by the instructor during the lessons.

Emergency Medical Treatment:
In the event of an emergency, participant gives permission for medical treatment to be administered as deemed necessary by the instructor or medical personnel. Participants understand that they will be responsible for any medical expenses that may arise.

Photography and Media Release:
I grant permission for the Kick Start Academy to take photographs or videos during the soccer lessons for promotional purposes and understand that images may be used in various media without compensation.
